Individual Support Plans (ISP)
An Individual Support Plan is a written tool that outlines an individual’s desired personal outcomes and the steps to achieve them. ISPs actively involve various people, services, and supports to help the individual meet their goals.
EDSI detailed plans involve:
Accessibility ensures that individuals with disabilities can access services and facilities without barriers. Within the ISP, it explains and outlines how EDSI has adapted their surroundings to meet their needs, teaching transportation skills and/or making transportation safer, and teaching different ways to communicate, accommodating diverse needs.
Safety and security are critical for individuals with disabilities. Within the ISP, EDSI explains how the individual is learning about safe environments to prevent accidents, injuries, and abuse. It includes teaching Fire prevention and safety, appliance safety, traffic safety etc all to the best ability of the individual-supported.
Accountability ensures responsible and transparent care. EDSI is accountable with the individual-supported for their actions, decisions, and outcomes. EDSI ensures tracking of service delivery, adhering to standards, and involving individuals and their families in care planning. These goal outcomes are within the ISP itself. The ISP is updated annually, and goal outcomes are reviewed bi-annually with goal updates quarterly.
Sustainability at EDSI involves balancing present needs with future well-being. It includes efficient resource use, environmental considerations, and financial stability. The ISP will include how the individual-supported is learning skills relating to sustainability measures.
Components:
- Personal Outcomes: These are the desired results or improvements the individual wants to achieve (e.g., independence, social connections, employment, etc).
- Goals: ISPs define specific objectives related to the outcomes.
- Action Steps: The plan outlines practical steps to reach each goal.
- Collaboration: ISPs involve family members, caregivers, professionals, and the individual themselves.
Measurable Progress:
- We track progress using specific indicators and quantify results.
- Our goal tracking system aims to be the best in the field.
- We provide frequent and transparent goal outcomes to the individual-supported and their care team.
- Goals are set collaboratively, and we celebrate achievements along the way.
Guidance and Support:
- Our staff provides guidance, encouragement, and positive reinforcement.
- We create a supportive environment where residents feel safe to explore and learn.
Individualization:
- Recognizing that each person’s journey is unique, we tailor our approach.
- Personalized strategies address specific strengths and challenges.
