Home/Hospital to EDSI (Group Living):

The transition plan addresses the logistics of the move from the person’s current home or hospital setting to an EDSI community home care location.

Goals might include adjusting to communal living, learning about the individual’s needs and preferences, building social connections, and participating in daily activities.

Supports involve EDSI staff, behaviour therapists, and other circle of care providers.

EDSI to Permanent Placement:

As the person progresses, or a space becomes available, the plan shifts toward permanent placement.

Goals may include learning about their new permanent placement through the use of social stories, securing volunteer or employment possibilities, and maintaining health and well-being.

The team coordinates services from agencies, pharmacies, new community groups and current health care team.
